About the Role:

As Nurse Practitioner Team Lead for our AI Care programs, you'll operate as a player-coach — delivering direct patient care while leading the recruiting, onboarding, development, and performance of the NPs in your pod.

What You'll Do:

  • Lead your pod: Manage clinical and professional performance, coaching, and corrective action for NPs in your pod, in partnership with Clinical leadership. Oversee visit volume, scheduling, and utilization.
  • Hire and onboard: Conduct clinical interviews for Circadia's NP pipeline and lead onboarding for new NPs.
  • Run 1:1s: Meet regularly with full-time NPs and per-diem NPs.
  • See patients: Maintain a direct patient-care panel of 8–16 hours/week, targeting 250 completed visits per month.
  • Report and document: Provide reporting and documentation for Clinical and executive leadership.
  • Improve the platform: Share clinical insight with Product, Engineering, and ML to strengthen Circadia's AI Care platform.
  • Support growth: Partner with Customer Success on clinical education for partner facilities; help design new clinical programs as our care model expands; support clinical implementation for new partner facilities.
  • Maintain quality: Author and maintain clinical SOPs; conduct chart review and quality oversight across the pod.
  • Flex as needed: Take on additional responsibilities as the AI Care organization scales.

Must-Have Qualifications:

  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute
  • Eligible for Medicare participation; company bills on your behalf
  • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently
  • 2+ years in a lead or people management role, and/or experience with clinical recruiting, onboarding, or program design
  • Comfortable conducting telehealth visits and navigating virtual care platforms
  • Thrives in a dynamic, evolving startup environment
  • Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools
  • English fluency required

Compensation:

Compensation & Perks

Base Salary: $160K-$180K
