About the Role:

This is a meaningful opportunity for a Patient Care Coordinator who wants to make a direct impact in home-based care. As Circadia's Patient Care Coordinator, you'll be the link between technology and patient care — traveling to patient homes across Orange County (including Mission Viejo and Anaheim) to install our remote monitoring devices, take patients' vitals, and facilitate initial telehealth visits with our Nurse Practitioners.

Your work enables earlier clinical intervention and helps prevent avoidable hospitalizations, directly advancing Circadia's mission to bring high-quality, proactive care into patients' homes.

Location(s): Anaheim, Mission Viejo, Irvine 

What You’ll Do:

  • Take and record patient vitals during home visits, ensuring accurate readings are captured and transmitted through the Circadia platform
  • Oversee installation of Circadia devices at designated patient locations for our Home Health Program
  • Facilitate patients' initial telehealth visits with our dedicated Home Health Nurse Practitioner
  • Educate and empower patients to build comfort using the device and setting up telehealth visits
  • Manage routine device maintenance, IT setup, configuration, and assembly of medical equipment
  • Coordinate with internal teams throughout the customer lifecycle to ensure successful technology deployment 
  • Audit and maintain the asset management list
  • Support the Procurement Team with supply chain planning for future installs and company growth
  • Come into the office once a week, or as needed, to pick up supplies

Must-Have Qualifications:

  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required, with demonstrated experience taking and recording vital signs (blood pressure, pulse, respiration, temperature, pulse oximetry)
  • Skilled with power tools and equipment for mounting and installation
  • Valid driver's license and reliable transportation (required for daily travel between patient homes)
  • Comfortable working independently with minimal supervision
  • Strong communication and customer service skills, especially with elderly or vulnerable populations
  • Familiarity with HIPAA and patient privacy standards
  • Willingness to pass a background check
  • Mission-driven and passionate about helping patients get the proactive care they need
  • Bilingual (Spanish) a plus, given the Orange County service area

Compensation:

As a full-time Patient Care Coordinator, you will be employed by Circadia Health, Inc. The anticipated hourly range for this part-time position is $30-$35/hour. The hourly rate is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
