About the Role

Circadia Health is expanding its clinical delivery infrastructure and is establishing physician-owned professional corporations (PCs) in multiple states, supported by a management services organization (MSO). We are seeking experienced physicians to serve as the owner and President of one or more affiliated medical groups. This is a governance and clinical leadership role — not a full-time clinical position — with responsibility for clinical oversight, regulatory compliance, and professional supervision of an NP-led care model serving skilled nursing and home health patients.

What You'll Do:

  • Serve as sole (or majority) shareholder, director, and President of one or more state-specific professional corporations
  • Provide clinical governance: approve clinical protocols, quality standards, and scope-of-practice policies for NP-delivered care (remote patient monitoring, chronic care coordination, transitional care)
  • Execute and maintain collaborative practice / supervision agreements with nurse practitioners as required by state law
  • Oversee credentialing, peer review, and clinical quality committees
  • Serve as (or designate) Medical Director for applicable programs; sign off on clinical policies, standing orders, and care pathways
  • Maintain Medicare enrollment for the PC and reassignment of billing rights consistent with program requirements
  • Partner with the MSO on compliant operations — while retaining exclusive authority over all clinical decisions, consistent with corporate practice of medicine (CPOM) requirements
  • Participate in periodic governance meetings, audits, and payer/regulatory inquiries as needed

Qualifications:

  • MD or DO with active, unrestricted medical license; willingness to obtain licensure in additional target states (licensure support and costs covered)
  • Active, unrestricted DEA registration
  • Board certification (Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine & Rehabilitation preferred)
  • Current Medicare enrollment (or eligibility) with no revocations, opt-outs, or payment suspensions
  • Clean compliance profile: no OIG/SAM exclusions, no state board discipline, no material malpractice history
  • Experience supervising NPs and/or serving as Medical Director in post-acute, SNF, home health, telehealth, or value-based care settings
  • Comfort with friendly-PC arrangements, including stock transfer restriction / succession agreements and MSO management structures

Nice to haves:

  • Prior experience as a PC owner for a telehealth, RPM, or digital health company
  • Multi-state licensure (especially CA, TX, NY, NJ, OH, and other CPOM-strict states) or IMLC participation
  • SNFist / post-acute medical director background; familiarity with PDPM, RPM/CCM billing (99453/99454/99457/99458), and CMS Conditions of Participation
  • Experience with quality programs, UM/peer review, and payer credentialing

Compensation:

Monthly retainer starting at $4,000/month (fair-market-value based), scaling with scope, number of entities/states, and time commitment

Additional compensation for clinical oversight time beyond baseline scope

Full indemnification, D&O coverage, and professional liability coverage provided

No capital contribution required; ownership subject to customary transfer restriction and succession agreements
